HealthyMe services are provided free of charge. Since eligibility requirements vary by program and provider, HealthyMe’s trained staff will connect you with appropriate resources and make sure that you have everything you need to qualify for specific services and programs.

We offer comprehensive services, including:

Health Connections to a wide range of services, helping you access resources, treatment, and care, including behavioral health, and specialized women’s health navigation in conjunction with The Promise Fund of Florida.

Social Determinants of Health (SDOH) navigation services to help bridge the gap between healthcare and the essential social care that impacts your health—such as housing, food, transportation, and legal assistance. 

A Resource Hub where you will find healthy living programs and visiting organizations providing onsite services at HealthyMe’s DiMino Center to support your overall well-being.

Medical and Ancillary Care Coordination and Urgent Dental Referrals

Our trained coordinators will assess your situation and provide appropriate options for you to establish a medical home and access the care you need.

Hospital Discharge Assistance Program

HealthyMe is pleased to offer a unique program to facilitate connections to resources, services and supplies for patients who are anticipating discharge from an acute care setting and lack insurance coverage or financial means necessary to maximize their chances of a successful recovery. We work closely with hospital case managers to connect eligible patients with services and resources to help them along their recovery to good health. A hospital referral is required for this program.

Women’s Health Navigation and Education

HealthyMe’s Women’s Breast and Cervical Cancer Navigation Program, supported by Promise Fund of Florida, provides outreach, education and access to screening, early detection and treatment to reduces disparities and improve health outcomes related to breast and cervical cancer. Eligible clients will be referred for appropriate services free of charge including screening mammograms, diagnostic and treatment services when needed as well as transportation assistance. All Promise Fund Clients will be connected to a Medical Home and screened and referred to behavioral health counseling services and social care services such as food, rent, utilities, legal assistance if needed

Behavioral Health Counseling and Referrals

HealthyMe’s Behavioral Health Program offers help for adults, teens, and children (ages four and over). Our services include a bi-lingual Licensed Clinical Social Worker (English and Spanish) who is trained in helping people improve their mental health and thrive in challenging times. We provide individual, family and couples therapy, culturally responsive treatment and trauma-informed care, parenting coaching, living with chronic diseases, play therapy, and evaluation and referrals for treatment of substance use and eating disorders. Sessions are available in person or via Telehealth, by appointment only.

Healthy Living and Visiting Organization Programs

HealthyMe offers a variety of nutrition, health, and wellness programs onsite in collaboration with local partners, including healthy eating, nutrition classes, supplies and education related to chronic disease management, and active living opportunities. We also host community-based organizations onsite to improve access to resources and services in the northern Palm Beach County area. Our on-campus DiMino Family Center is a hub for community health and wellness education thanks to The Frank DiMino Family Foundation.   • HealthyMe is pleased to offer "Healthy for Life," a four-week community nutrition initiative aimed at fostering sustainable, healthful eating and mindfulness practices. Led by HealthyMe’s Nutrition Education Provider, the program utilizes an evidence-based curriculum created the American Heart Association and Aramark and incorporates “Healthy for Life” lesson plans, strategies for making healthy food choices within budget. The program also includes education related to Mind/Body/Food connection facilitated by HealthyMe’s Behavioral Health provider. The highlight of the program is a guided grocery shopping tour where participants gain hands-on experience in reading labels, making economical food purchases and maximizing resources such as SNAP benefits. Social Care Connections

HealthyMe’s trained Care Coordinators take the time to identify individual and family needs and provide information and linkages to the following services:

  • Food and nutrition resources

  • Housing, rent and utilities assistance

  • Transportation to health-related appointments

  • Legal assistance 

  • Benefits assistance

  • Family support

Our dedicated staff and volunteers will reach out to you at regular intervals to make sure that you received these services. We will continue to work with you to create a road map to help you and your family achieve your health and wellness goals and independence.
